Social Care Network Coordinator

Our programs are staffed by experts who help over 325,000 clients each year and advocate on behalf of all needy New Yorkers. Our services include 100% affordable housing at 21 locations, family violence services, Holocaust survivor assistance, geriatric social work, crisis intervention and the largest free kosher food distribution program in the world. Our network of 120 food pantries, affordable housing sites, and JCCs provide services directly in neighborhoods across New York City.

Position Summary:

As a service provider in the new Medicaid-funded Social Care Network (SCN), administered by Public Health Solutions and covering Brooklyn, Manhattan, and Queens, Met Council will receive client referrals and ensure the timely delivery of contracted services through the SCN. Met Council’s Food Program will provide grocery and cooking supplies via home delivery, and the Geriatrics Program will provide home accessibility and safety modifications, as well as home remediation assistance. The services are in step with current services provided by Met Council, though referrals will now be coming from navigators external to Met Council, and Medicaid reimbursement for these services will be available.

The SCN Coordinator will receive referrals, coordinate with the appropriate team for service deliveries, and respond within the designated timeframe according to the SCN operating procedures. All referrals and reimbursement requests will be submitted using UniteUs, a digital tool for managing such tasks. The SCN Coordinator will work with the Salesforce Admins at Met Council to support the integration of UniteUs with Salesforce.

This role works with multiple teams, but reports to the Director of Strategic Initiatives for the Food Programs team.

Please note: This is a hybrid position requiring on-site attendance three days per week at Met Council’s main office located in the Financial District. Normal work hours are Monday – Thursday, 9am - 6pm, and Friday, 9 am - 3pm.

Principal Responsibilities:

  • Coordinate client referrals and fulfillment of service requests from the SCN to the appropriate Food Pantry and SenioRepair program within the required service timeframes.
  • Work with individual departments to identify and connect eligible clients to the SCN for navigation to Food Pantry and SenioRepair services.
  • Monitor communication channels to support the referrals of services provided by Met Council Food Programs and SenioRepair team.
  • Facilitate referral workflows using the UniteUs platform. Complete relevant trainings to learn the platform, and onboard fellow Met Council team members, when relevant.
  • Join regularly scheduled webinars and meetings for Social Care Network service providers, keep up with developments and report them out.
  • Work with the Salesforce Admins for Food Programs and Social Services to support future UniteUs<>Salesforce integrations.
  • Additional responsibilities as required.

Skills and Education:

  • Bachelor’s degree (preferred)
  • Fluent in English (both written and verbal)
  • Fluency in other languages is valued
  • Familiarity with Salesforce, UniteUS, or similar CRM platforms (preferred)
  • Strong knowledge of Office 365 and its applications
  • Comfort with technology (required)

Compensation: $45,000 - $55,000 per year.
